The form should be sent to the supervisor or mentor. There they can provide comments about the document. At the end of that, they will add the signatures before printing the final draft.

Sometimes a situation arises where a report can't be sent electronically, the mentor or supervisor has to handwrite their comments. Here, the report gets signed by the supervisor and mentor and can be scanned and mailed back to the member-in-training.

 In order to make the comments confidential, the mentor now can send the signed report directly to APEGS. Finally, the member-in-training has to print the signed report and sign page 1 where needed.
